After you upload the file please copy the exact file Name, then
type or copy the same in Text Box on the main form with the file
extension (e.g: Manscript.doc, Manscript.pdf or Manscript.zip )

If you are sending multiple files then send them in zip folder and type
the name of your file with zip extention as shown above in the text box.
